系统性红斑狼疮

概述
系统性红斑狼疮是一种较常见的累及多系统多器官的自身免疫性疾病,由于细胞和体液免疫功能障碍,产生多种自身抗体。发病机理主要是由于免疫复合物形成。确切病因不明。病情呈反复发作与缓解交替过程。本病以青年女性多见。我国患病率高于西方国家,可能与遗传因素有关。

病因
病因尚不清楚,可能与多种因素有关。
一、遗传:发病有家族倾向。
二、感染:未能证实。
三、激素:大部分SLE患者为育龄妇女,男女之比至少为1∶7-9。无论男女患者血中雌酮羟基化产物皆增高。
四、环境:日光和紫外光照射能使SLE全身和皮肤症状加重,日光过敏见于20-40%患者。日晒后出现颊、额、颈、胸、手背等处红斑。
五、药物:服用某些药物如普鲁卡因酰胺,肼苯哒嗪可引起药物性狼疮,其症状与自发SLE相似,停药后症状和自身抗体消失。

症状
一、全身症状:起病可急可缓,多数早期表现为非特异的全身症状,如发热,尤以低热常见,全身不适,乏力,体重减轻等。病情常缓重交替出现。感染、日晒、药物、精神创伤、手术等均可诱发或加重。
二、皮肤和粘膜:皮疹常见,约40%患者有面部典型红斑称为蝶形红斑。急性期有水肿、色鲜红,略有毛细血管扩张及鳞片状脱屑,严重者出现水疱、溃疡、皮肤萎缩和色素沉着。手掌大小鱼际、指端及指(趾)甲周红斑,身体皮肤暴露部位有斑丘疹、紫斑等。出现各种皮肤损害者约占总患病数的80%,毛发易断裂,可有斑秃。
三、关节、肌肉:约90%以上患者有关节肿痛,且往往是就诊的首发症状,最易受累的是手近端指间关节,膝、足、髁、腕关节均可累及。关节肿痛多呈对称性。 约半数患者有晨僵。X线检查常无明显改变,仅少数患者有关节畸形。肌肉酸痛、无力是常见症状。
四、肾脏:约50%患者有肾脏疾病临床表现,如蛋白尿、血尿、管型尿、白细胞尿、低比重尿、浮肿、血压增高、血尿素氮和肌酐增高等。
五、心脏。
六、肺:肺和胸膜受累约占50%,其中约10%患狼疮性肺炎,胸膜炎和胸腔积液较常见。
七、神经系统:神经系统损害约占20%,一旦出现,多提示病情危重,大脑损害可出现精神障碍,如兴奋、行为异常、抑郁、幻觉、强迫观念、精神错乱等。
八、血液系统:几乎全部患者在某一阶段发生一项或几项血液系统异常,依次有贫血、白细胞减少、血小板减少、血中抗凝物质引起出血现象等,贫血的发生率约80%,正细胞正色素或轻度低色素性。


检查
一、一般检查:病人常有贫血,白细胞和血小板减少,或表现为全血细胞减少。血沉常增快。肾损害者有程度不等的尿检查异常,如蛋白尿、血尿。血浆蛋白测定可见球蛋白增高,特别在有肾变性肾炎时,白/球蛋白比例倒置,血胆固醇增高,严重肾损害者血中尿素氮和肌酐升高。
二、免疫学检查:血中存在多种自身抗体是其特点,抗核抗体(ANA)在病情活动时几乎100%阳性。阴性时更换检查方法可能出现阳性,抗核抗体阴性时不能完全排除本病,需结合临床和其他实验室检查资料综合分析。抗双链DNA(ds-DNA)抗体对诊断的特异性较高,但阳性率较低,为40-75%,与疾病活动和肾脏损害密切相关,抗体效价随病情缓解而下降,抗Sm抗体约在30%SLE中呈阳性反应,因其特异性高,又称为本病的特异性抗体。对于不典型、轻型或早期病例,按SLE标准不足确诊者,若抗Sm抗体阳性,结合其他表现可确诊。

治疗
1、一般原则:急性活动期卧床休息,积极治疗感染,积极治疗并发症。
2、药物治疗:
1)非类固醇抗炎药。消炎痛效果较好,其镇痛、退热及抗炎作用都较强。
2)抗疟药:对于控制皮疹、光敏感及关节症状有一定效果,是治疗盘状狼疮主药。
3)皮质激素:目前治疗主要用药。通常用泼尼松,长期应用副作用较多,易复发。
4)雷公藤:具有消炎、抗菌、调节免疫、活血化瘀等作用。
5)免疫抑制剂:对本病有一定疗效,常用者有环磷酰胺、硫唑嘌呤。

What is Feng Shui

For the YIN Dwelling, people look for a burial ground for their dead parent(s) / grandparent(s), the right selection of sites benefits them and influences their future and/or fate of their descendants. However, following by the increase of population of human beings and restrictions of burial grounds, finding a distinctive, isolated ground that suit most of the YIN Dwelling Feng-Shui requirements with ample front and back surrounding space is increasingly difficult.

For a suitable burial site, the Environment Feng-Shui and Flying Stars practices are the key theories to rely on. The factors under consideration are:

DRAGON:

The trend of aggregation and dispersion of the lively force of landscape (mountains and water)

XUE:

In the YANG Dwelling category, all three theories are relevant in terms of the selection of a good site for individual dwellings. A good house is a place where people live in comfortably and which is able to magnify your good fortune. Flying Stars Theory emphasizes on the continuing changes of effects Fung-Shui has on the dwelling which affect the living individual who dwells in it. Since the stars in the cosmic space circulate around the orbits and interacting forces continue to affect our environment, the Feng-Shui optimal directions will also vary with time as stated in Sim's Metaphysics Theory. Nothing is fixed and everything is influenced by the switching Nine Flying Stars and the coupling effect of the stars of backing and facing directions of your house.

Time dimension relating to Feng-Shui is divided into 3 phases: upper, medium, and lower phases (each phase means 180 years). Each phase is sub-divided into nine bi-decades (nine 20-years). Year 1999 is within the lower phase 7th bi-decade.

The Eight Housing, stresses on a static Feng-Shui effect on individual depending on one's birth year. The theory classifies people into 2 types: east four life and west four life. Different life type has different Feng-Shui effects in the main eight directions. And the effects descend in the following order:

Chinese Chong Yang Festival

The 9th day of September in the lunar calendar is the Chinese Chong Yang Festival and a happy occasion in autumn. According to the traditional theory of Yin and Yang, both the 9th month and the 9th day of the month belong to Yang, which means positive and masculine, and Chong means double, thus it is called Chong Yang. People often gather for a party, appreciate chrysanthemums, pin the leaves of Cornus on clothes. And the custom of climbing mountains and eating a special cake also features the day.

This custom of climbing mountains can be dated back as early as West Han Dynasty. Some old travel notes have it that, people climbed to mountain peaks not only for beautiful scenes and poetic inspiration, but also the avoidance of evil spirits and disasters.

This practice came from an ancient folktale. It is said long ago there appeared a devil of plague in the Ruhe River. People lay down and died wherever it came up. A boy named Heng Jing swore to help his neighbors and fellow people to get rid of it. He visited many famous mountains to seek a powerful master.

Finally an old Taoist took him in and taught him how to defeat the devil. Heng Jing put his whole heart into study and practice. One day, the Taoist called him up and said, 'Heng Jing, tomorrow is the 9th day of September and the devil will reappear. It is time for you to go home and stop the devil.' The master also gave him a pack of leaves of Cornus and a jar of liquor soaked with chrysanthemums.

Riding a crane, Heng Jing went a great distance back home in a day. As instructed by his master, he told his fellow villagers to climb up the nearby mountain with a Cornus leaf pinned on their clothes and a glass of chrysanthemum liquor in hand. When the devil of plague came up from under the water, it got dizzy by the scent of Cornus and chrysanthemums.

Heng Jing fought with his master's sword and killed the devil in a few rounds. People held parties, drank chrysanthemum liquor to celebrate it. And the next year, the custom of mountain climbing became popular among the villagers.

In the golden September, chrysanthemum blooms, reminding people of the folktale. And later a special cake with dates, chestnuts and meat was made to add more festivity to the special day.

Lion Dance

Bringing luck and happiness The lion dance is an important tradition in China. Usually the dance is part of festivities like Chinese New Year, the openings of restaurants and weddings.
If well-performed, the lion dance is believed to bring luck and happiness. Although lions are not native in China, they came to this country via the famous Silk Road. Rulers in what is today Iran and Afghanistan sent lions to Chinese emperors as gifts in order to get the right to trade with Silk Road merchants. The lion dance dates back to the Han Dynasty (205 B.C. to 220 A.D in China) and during the Tang Dynasty (716-907 A.D.) it was at its peak. It was particularly performed during religious festivals. The lion dance was not only introduced in China, but also in Korea and Taiwan, where lions are not native as well. The dances are not exactly the same in these countries, but the symbolism is quite similar.The lion is enacted by two dancers. One handles the head, made out of strong but light materials like paper-mache and bamboo, the other plays the body and the tail under a cloth that is attached to the head. The 'animal' is accompanied by three musicians, playing a large drum, cymbals and a gong. A Little Buddha teases it with a fan or a giant ball. The head dancer can move the lion's eyes, mouth and ears for expression of moods.The lion dance combines art, history and kung fu moves. Normally the performers are kung fu practitioners. Every kind of move has a specific musical rhythm. The music follows the moves of the lion: the drum follows the lion, the cymbals and the gong follow the drum player.Quite often people observing the dances think that they are looking at dragons. Recipe -

Sweet and Sour Pork.
Serves: 6

This sweet and sour pork recipe is easy to make in a skillet, with pineapple and barbecue sauce.

Ingredients :

1 1/2 pounds boneless pork shoulder or country style ribs, cut in small cubes
1 tablespoon vegetable oil
1 teaspoon salt
dash pepper
1 can (about 8 1/2 ounces) pineapple chunks or tidbits
1/2 cup barbecue sauce
1 tablespoon cornstarch
2 tablespoons cold water
1 green bell pepper, cut in strips (I use half red bell pepper for color)

Method :

In a heavy skillet over medium heat, brown pork cubes in the vegetable oil. Season with salt and pepper. Drain pineapple syrup into a 1-cup measure and add enough water to make 3/4 cup. Set the drained pineapple aside. Combine syrup and water mixture with barbecue sauce; pour over the meat in the skillet. Cover and simmer for 40 to 45 minutes, or until pork is very tender. Combine the cornstarch with the cold water, stirring until smooth. Add to pork mixture and stir, cooking, until thickened. Add bell pepper strips and reserved pineapple chunks and heat through. Serve over rice.


Won Ton Soup
Serves: 10

Ingredients :

1/2 lb. ground pork
1/2 Tbls. minced green onions
4 oz. can tiny shrimp - drained, chopped
1 1/2 Tbls. minced fresh ginger
1 1/2 tsp. sesame oil
1 egg white - lightly beaten
1 Tbls. soy sauce
1/2 tsp. salt
1/4 tsp. white pepper
1 1/2 Tbls. cornstarch
12 oz. package won-ton wrappers
1 egg - beaten
3 qts. water - for boiling

Method :

-Using your hands or a spoon, combine pork, shrimp, ginger, sesame oil, egg white, salt, soy sauce, and pepper.
-Add the cornstarch to mixture and stir to blend.
-Place a scant teaspoonful of the filling in the center of each wonton skin.
-Fold one corner over the filling toward the opposite corner. Roll the wonton up diagonally towards the corner you just folded in. Brush with egg. Fold the two remaining corners into the middle. Place the finished dumplings on a tray that has been lightly dusted with cornstarch.
-Heat 3 qts. of water to boiling in a large pot. Add the won tons and cover. Once the water is boiling again, cook for about 4 minutes or until they rise to the surface. Remove with a long-handled strainer and drain. Discard the water.
-Heat remaining ingredients to boiling, add the cooked won-tons, and serve.
